Each RN will be assigned one borough and will primarily serve members within that area. This hybrid position includes remote flexibility with one field day per week for in-home member visits. RN Case Manager Position Details by Location: • Bronx: Bilingual Spanish preferred • Brooklyn: Bilingual Chinese, Spanish, or Russian preferred • Manhattan: Bilingual Cantonese or Mandarin preferred RN Case Manager Key Responsibilities: • Manage a dedicated caseload within one borough, ensuring members receive appropriate and accessible care. • Develop, oversee, and update individualized care plans to align with members’ evolving needs. • Conduct home visits (one borough, 4-6 visits per week) to assess home safety and care needs. • Collaborate with leadership and Intensive Care Management teams to address complex cases. • Present high-risk cases for multidisciplinary review. RN Case Manager Qualifications: • Active New York State RN License required. • Bachelor’s degree in Nursing, Health, or Human Services preferred. • Minimum two (2) years of home care experience required. • Strong experience in care planning, patient assessments, and home safety evaluations. • Prior supervisory experience is a plus. • Bilingual candidates strongly preferred (see language preferences by borough above).
