This is not a clinical caseload role. Instead, the focus is on leveraging established professional relationships within the MCO system to generate referrals for Medicaid and waiver clients who need follow-up home care services. The ideal candidate is well connected to case managers, transition-of-care specialists, and discharge planners, and can use those relationships to develop referral streams for the agency's home care programs.
