Referral Development Manager - NJ Medicaid (Home Care) About the Role A multi-site home care agency in New Jersey is seeking a Referral Development Manager with strong, hands-on experience inside one of New Jersey's major Managed Care Organizations (MCOs) such as Aetna, Amerigroup, Horizon NJ Health, UnitedHealthcare Community Plan, or WellCare. This is not a clinical caseload role. Instead, the focus is on leveraging established professional relationships within the MCO system to generate referrals for Medicaid and waiver clients who need follow-up home care services. The ideal candidate is well connected to case managers, transition-of-care specialists, and discharge planners, and can use those relationships to develop referral streams for the agency's home care programs. Key Responsibilities • Build and maintain referral pipelines from MCOs, hospitals, rehab centers, and provider networks. • Leverage existing professional contacts within NJ MCOs (case managers, transition coordinators, discharge planners) to generate qualified client referrals. • Serve as the key liaison between the home care agency and MCO care teams to ensure a smooth referral and onboarding process. • Partner with hospital discharge planners, rehab facilities, and community organizations to identify eligible Medicaid and waiver clients. • Track referral activity, outcomes, and growth metrics; report performance to leadership. • Represent the home care agency in meetings and networking opportunities to expand referral channels. Qualifications • Minimum 3-5 years of experience working within or directly alongside NJ MCOs in a case management, care coordination, or provider relations role. • Strong, active referral relationships with discharge planners, transition-of-care specialists, and case managers across NJ. • In-depth knowledge of NJ Medicaid programs, waiver services, and MLTSS. • Proven ability to drive business development or referral growth in healthcare settings. • Excellent communication, networking, and relationship-management skills. • Bachelor's degree in healthcare, social work, business, or related field preferred; equivalent experience considered. Compensation & Work Arrangement • Base salary: $90,000-$100,000 • Quarterly incentive/bonus structure with OTE up to $150,000 • Primarily remote, with occasional in-office presence and statewide travel as needed
