The Care Team Assistant provides clinical and clerical support to ensure quality and compassionate care in a virtual environment. Responsibilities include managing communications with caregivers and community partners, maintaining patient lists, and supporting outreach efforts. | Candidates should have a high school education or equivalent, along with a Medical Assistant Certification or at least 2 years of clinical experience. What You’ll Do The Care Team Assistant provides both clinical and clerical support and ensures the provision of quality and compassionate evidence-based care in a virtual Value-Based Care environment. In this position, you will: Accept incoming phone calls and return calls from care givers and community partners inquiring to see if patients are eligible for our program or have questions about Imagine Pediatrics Pull daily list for TOCs and work closely with the Outreach RNs Create TOC hospital specific list to send daily to hospital partners Place calls and create template-based letters, as directed by Outreach and Network leadership Review schedule on a weekly basis to ensure that all patient lists and PCP reports are pulled for Network team Input new endorsement or PCP specific instructions into Care Hub Set up PCP meetings by working closely with clinical teams to build off existing relationships with community providers Ability to explain the Imagine program and enroll caregivers/child if needed Perform other duties as assigned What You Bring & How You Qualify First and foremost, you’re passionate and committed to reimagining pediatric health care and creating a world where every child with special health care needs gets the care and support they deserve. What You’ll Do As an Associate Data Product Manager you will support senior product and project managers, analysts, and data engineers by responding to requests for data and features, analyzing healthcare information and delivery datasets to create business intelligence, helping to define features, and generally supporting the work of data-informed decision-making. You will: • Be a frontline responder to data analysis requests that support the success of our healthcare delivery and business support teams. • Partner with senior product and project managers to define data product requirements and technical specifications, prioritize work efforts, and drive product enhancements. • Blend and analyze operational business and healthcare data to identify trends, answer questions, and define areas for improvement. • Collaborate with cross-functional teams to gather requirements and ensure successful data and product delivery. • Contribute to data quality assurance initiatives and activities. • Perform other duties as assigned. What We Offer (Benefits + Perks) The role offers a base salary range of $75,000 - $100,000 in addition to annual bonus incentive, competitive company benefits package and eligibility to participate in an employee equity purchase program (as applicable). What You’ll Do The Care Team Assistant provides clerical support and ensures the provision of quality and compassionate evidence-based care in a Value-Based Care environment. In this position, you will: Accept incoming phone calls and return calls from patients involving clinical inquiries for responsible providers, perform protocol-based triage as appropriate, and route phone calls in the form of a phone note document in the EHRS to the provider for review and instructions Place calls and create template-based letters, as directed by the provider, regarding all lab results for delivery to the patient in a timely manner Review schedule on a daily basis to ensure that all pre-visit preparations have been completed prior to the patient's visit, ensuring lab and imaging results, consultation reports, care transition details, or major changes in health status are available in the patient’s medical record; when these documents are not present in the record, it will be the Care Team Assistant’s responsibility to call the patient, consultants, imaging centers, and/or hospitals to arrange for timely delivery of said information Call patients deemed to be at risk for a “no-show” prior to their appointment as well as new patients establishing care for their onboarding visit to remind them of both the patient’s and Imagines’ responsibilities; contact "no-show" patients and inquire as to their status and need for rescheduling, including assessment of barriers to care and initiation of appropriate social service referrals Monitor and respond to questions and escalations in CareHub and other internal platforms Review TOC lists and assist with determining discharge dates Contact patients to schedule follow-up appointments Monitor and respond to questions and escalations in Teams chats Assist care team in regular completion of reports that will allow it to manage and assess the health needs of the patient population; assist care team in recognizing patients who are members of a "vulnerable population" and provide appropriate social service or community-based referrals. Assist care team and coordinators with data tracking and caseload tracking Collaborate with colleagues and care team members virtually Perform other duties as assigned What You Bring & How You Qualify First and foremost, you’re passionate and committed to reimagining pediatric health care and creating a world where every child with complex medical conditions gets the care and support they deserve. In this role you will need: High-school level education or equivalency At least 2 years of clinical experience in a behavioral health inpatient, outpatient, ambulatory clinic, or virtual setting assisting with record management and requests. Preferred working knowledge of the hospital setting and discharge planning process Medical Assistant Certification preferred Spanish fluency preferred Demonstrated competency with an EHRS systems This job requires access to confidential and sensitive information, requiring ongoing discretion and secure information management.
