Who We Are Imagine Pediatrics is a tech enabled, pediatrician led medical group reimagining care for children with special health care needs. We deliver 24/7 virtual first and in home medical, behavioral, and social care, working alongside families, providers, and health plans to break down barriers to quality care. We do not replace existing care teams; we enhance them, providing an extra layer of support with compassion, creativity, and an unwavering commitment to children with medical complexity. What You’ll Do The Care Team Assistant provides both clinical and clerical support and ensures the provision of quality and compassionate evidence-based care in a virtual Value-Based Care environment. In this position, you will: Accept incoming phone calls and return calls from care givers and community partners inquiring to see if patients are eligible for our program or have questions about Imagine Pediatrics Pull daily list for TOCs and work closely with the Outreach RNs Create TOC hospital specific list to send daily to hospital partners Place calls and create template-based letters, as directed by Outreach and Network leadership Review schedule on a weekly basis to ensure that all patient lists and PCP reports are pulled for Network team Input new endorsement or PCP specific instructions into Care Hub Set up PCP meetings by working closely with clinical teams to build off existing relationships with community providers Ability to explain the Imagine program and enroll caregivers/child if needed Perform other duties as assigned What You Bring & How You Qualify First and foremost, you’re passionate and committed to reimagining pediatric health care and creating a world where every child with special health care needs gets the care and support they deserve. In this role you will need: High-school level education or equivalency Medical Assistant Certification or equivalent, or at least 2 years clinical experience in an outpatient setting Spanish fluency preferred Excel experience Eastern timezone preferred What We Offer (Benefits + Perks) The hourly rate for this position ranges from $21 - 24 per hour in addition to competitive company benefits package and eligibility to participate in an employee equity purchase program (as applicable).
