Responsibilities The Community Engagement Specialist works individually and is accountable for enrolling eligible patients into the HarmonyCares program. This position will work collaboratively with a Community Account Executive, to outreach physician offices and healthcare facilities, and schedule appointments to present the HarmonyCares program. Essential Duties And Responsibilities • Conduct thorough research to understand patient's healthcare needs, healthcare utilization and best number to call • Conduct telephonic outreach to eligible patients and caregivers to explain and enroll patients into the HarmonyCares program • Conduct telephonic outreach to physician offices, hospitals and skilled nursing facilities to help engage their patients into the HarmonyCares program • Understand HarmonyCares program in depth, explain the program to patients, caregivers and providers and handle objections creatively • Build relationships with community providers and facilities and collaborate with a cross-functional team to ensure delivery of a great patient experience • Leverage electronic tools, spreadsheets and reports to facilitate research, identify innovative approaches, and document and present your plans and progresS • Meet or exceed established monthly engagement targets Qualifications Required Knowledge, Skills and Experience • Associates degree, or 3+ years of progressive career growth • 2+ years of related experience in sales, patient engagement, community outreach or customer service, preferably in the medical/healthcare industry • Proficiency in Microsoft Excel, Word, and PowerPoint • An engaging personality. Great active listening, written and verbal communication skills • Detail oriented with excellent documentation skills. Organized and responsive with great follow-up • Compassion, an interest in people and a professional presentation and demeanor at all times • Curious and a continuous learner, a passion for solving problems with a get-it-done attitude Preferred Knowledge, Skills And Experience • Bilingual in Spanish, Mandarin or other language a big plus • Contact center experience a plus • Comfort with CRM and EMR tools a plus Posted Max Pay Rate USD $82,000.00/Yr. Posted Min Pay Rate USD $68,000.00/Yr.
