Establish and oversee medication management strategies aligned to guideline-directed medical therapy (GDMT) and population health best practices. Define and maintain clinical pharmacy practice guidelines to optimize outcomes and reduce avoidable costs. Value-Based Performance & Cost Management Drive medication cost containment strategies across Commercial, Medicaid, and Medicare Advantage contracts. Monitor utilization trends and partner with finance and analytics teams on forecasting and budget planning. Support performance optimization under value-based and shared-savings arrangements. Cross-Functional & Clinical Partnership Partner with Regional and Enterprise Medical Officers to operationalize medication-specific insights through Vytal Insights. Collaborate with Care Management, Operations, Technology, and Clinical Analytics teams to align medication strategies with care workflows. Support utilization management initiatives through data-driven insights and measurable KPIs. Vendor & External Collaboration Manage and collaborate with external pharmacy and medication-management vendors. Ensure vendor adherence to evidence-based protocols, regulatory requirements, and contract expectations. Performance Monitoring & Continuous Improvement Establish program metrics, monitor performance, and drive continuous improvement initiatives. Translate data and outcomes into actionable insights for executive and clinical leadership. Key Performance Indicators (KPIs) Medication Cost Management: Reduction in medication spend; achievement of cost targets across payer contracts. Clinical Outcomes: Increased adherence to GDMT; improved chronic disease outcomes; enhanced patient education and engagement metrics. Program Performance: Growth in Vytal Care program enrollment; care plan completion rates; quarterly improvement initiatives delivered. Vendor & Contract Compliance: Adherence to evidence-based protocols; achievement of value-based contract performance measures. Financial Forecasting & Analytics: Accuracy of medication cost forecasts; timely and actionable utilization reporting. What will make you successful here Minimum 10+ years of experience in clinical pharmacy, chronic disease management, or healthcare leadership roles. Demonstrated success managing medication strategies within value-based care or risk-based contracts. Deep understanding of payer dynamics, population health, and evidence-based medicine. Strong financial acumen with experience in cost management, forecasting, and performance improvement. Proven ability to lead cross-functional initiatives and influence clinical and operational stakeholders. Preferred Qualifications Advanced clinical degree (PharmD, MD, or equivalent).
