Conduct employment readiness assessments and develop individualized service plans to help veterans secure and maintain stable employment. | High school diploma required; bachelor's degree preferred with experience in human or social services. | We are seeking a compassionate and motivated Case Manager to support veterans in obtaining and maintaining meaningful employment. This role places a strong emphasis on employment retention, working closely with veterans after job placement to ensure long-term success in the workplace. The ideal candidate is relationship-driven, resourceful, and committed to helping veterans achieve lasting self-sufficiency through stable employment.   Key Responsibilities * Conduct comprehensive assessments to evaluate veterans’ employment readiness, skills, and career goals * Develop individualized service plans focused on job placement and long-term employment retention * Build and maintain relationships with local employers to identify job opportunities for veterans * Provide ongoing support to employed veterans to promote job retention, including regular check-ins, coaching, and problem-solving around workplace challenges * Coordinate with employers and community partners to support successful job placements and continued employment * Connect clients to supportive services such as healthcare, behavioral health, and workforce development programs * Monitor client progress and adjust service plans to support continued employment success * Maintain accurate and timely case notes and documentation * Advocate on behalf of clients to reduce barriers to employment retention Qualifications High school diploma with human services or social services experience.  Bachelor's degree preferred.
