In this role, you will partner with our PM&R physician in the sub-acute rehab setting. This position is perfect for those passionate about improving patient outcomes and working within an interdisciplinary, specialized team. Why Partner with Us? Expert Team: Collaborate with Board-Certified Physical Medicine and Rehabilitation Physicians along with other Nurse Practitioners and Physician Assistants trained in PM&R Flexible Schedule: Ability to balance life and work No On-Call: Evenings and weekends free from interruption Improving Outcomes: Help improve patient functionality and satisfaction in senior living communities On-Site Pain Management: Provide consistent care with a focus on reducing re-admission rates Coordination of Care: Work with medical providers, therapists, social workers, nurses, and families to ensure optimal patient care About the Role: Location: In-person at skilled nursing facilities Schedule: Flexible options, Part-time positions available Culture: Detail-oriented, outcome-focused, stable, people-oriented, and team-oriented Responsibilities: Partner with the facility's therapy department to provide the best possible rehab care.
