Essential Functions & Responsibilities • FT telemed providers are required to maintain a minimum of 5 licenses in states where Theoria actively provides care. At any time Theoria may request providers to apply for and obtain a new license at the expense of Theoria. Multi-state licensure is a requirement of the telemed position. • The provider is responsible for managing the clinical care of the population at the assigned SNFs. • The provider is responsible for monitoring message boards, answering calls, and communicating with assigned SNFs during shifts. • Provider will provide high-quality, patient-centered, comprehensive primary care services to patients residing in SNFs. • The provider is responsible for timely documentation of all patient care/encounters • The provider will be available for phone/secure messaging consultations with facility nursing staff. • Provider documentation is completed in Theoria’s proprietary electronic health record (EHR) system, ChartEasy™. • PointClickCare, MatrixCare, and American HealthTech are the most commonly used facility EHR systems. • The provider is expected to be ready and on time for their remote scheduled shifts. • Timely attendance is imperative. • Reliable Wi-Fi is required. Shift Structure • Full time day shift is Monday through Friday 1PM-7PM and 2 12-hour weekend shifts per month, 7AM-7PM, during which the provider must be available to nursing staff and address acute/emergent issues that arise. • Full time night shift is 15 shifts per month 7PM-7AM, during which the provider must be available to nursing staff and address acute/emergent issues that arise. • Shift times are in EST • Charting/documentation can be completed after the shift but within 48 hours • This is a remote position.
