Review patient data and clinical algorithms to develop and sign off on care plans for remote patient monitoring and chronic care management. | Must be a certified Nurse Practitioner licensed in at least 10 states with active, unrestricted licenses including TX, NJ, CA, and have knowledge of chronic care and RPM. | POSITION SUMMARY: This is a full time, completely remote role. Monday-Friday, standard business hours. Please list all states in which you are licensed in as an NP, either in your CV or in a cover letter. Do not apply if you are not licensed in at least 10 states. In this role, you will be responsible for reviewing pertinent patient data, developing care plans, recommending modifications to current treatments, tracking progress to goals, and signing off on the treatment plans once the review is completed. The output of your reviews will be used by onsite or telehealth providers to activate care plans and identify patients at risk for declining health status. Specifically, you will review records to enable Remote Patient Monitoring services (RPM) and Chronic Care Management (CCM) services. Primary Responsibilities • Review all curated patient data reports received for accuracy of clinical and demographic information; Review output of clinical care algorithms for accuracy and appropriateness in the context of each patients’ medical history, care plan goals, and medications. • Based on patient medical history and health status, develop care plans for the treating providers to implement • Review progress to care plans; identify patients at risk for a decline in health without greater adherence to care plans • Support Data Quality Reviewers, LPN Reviewers and RN Reviewers in building their clinical knowledge and clinical decision making to enhance the quality of the initial reviews • Coach APP new hires to help them in building their understanding of the role, tools, and technology • As needed, support initial clinical and demographic data review generated by the system to prepare and finalize final care plan sign off and billing. • Identify discrepancies in data and/or algorithm output; escalate through defined channels for correction. • Identify potential gaps in care to complete the review. • Communicate with Director of Clinical Review on issues regarding quality, system capacity, and resource requirements, facility and/ or provider feedback. • Meet established standards for productivity, quality, timeliness, and customer satisfaction • Performs all duties and responsibilities and adheres to the Nurse Practice Act. • Protects patient information and adheres to standard of confidentiality with all patient information • All other duties as assigned. Minimum Requirements • Nurse practitioner education and certification. • At least 10 state licenses, including TX, NJ, CA • Active and non-restricted license • Knowledge and Skill in chronic care management and remote patient monitoring management preferred. • Proficient with Adobe, Microsoft Word, Excel, and video conferencing platforms and Dashboard type platforms. • Strong verbal/written communication and presentation skills • Experience in working with team members to promote ideas, issues, and initiatives within a constructive group framework • Must always represent the organization in a positive and professional manner • Ability to organize and prioritize tasks • Ability to work independently, be attentive to detail and maintain a positive attitude. Provide hybrid onsite and telehealth administrative support to Advance Practice Providers by managing schedules, preparing patient documentation, facilitating chronic care management visits, and traveling up to 50% annually. | Must have CMA/CCMA/RMA certification or relevant medical assisting experience, reside in the Great Lakes territory, have reliable transportation, proficiency in Microsoft and EHR systems, and be willing to travel and comply with vaccination policies. | In this role, the Travel Medical Assistant will work closely with our Advance Practice Providers (APPs) to facilitate visits to patients in long term care settings. You will provide onsite and administrative support to our APPs in the daily management of their practices, helping them to maximize their impact on conducting initiating patient visits for chronic care management services. This role is a hybrid role with 50% annual onsite travel and 50% via telehealth providing administrative medical assistant work. PRIMARY RESPONSIBILITIES: • Support telehealth providers through a variety of tasks related to patient care management, organization, and communication. • Daily management of the assigned providers’ schedule to ensure appropriate coverage of facilities. • Prep the patient note and upload appropriate documentation to support chronic care patient visits. • Maintain health information in compliance with corporate and federal regulations. • Resolve issues that relate to clients, patients, operations, and workflow. • Frequent communication with providers, clients, etc., to ensure the success of the program/practice. • Execute onsite rounds with Telehealth provider using Tapestry issued equipment. • Provide patient education regarding the importance of chronic care management annual visits. • Obtain signed consent from patients for chronic care management. • Maintain a professional appearance with Tapestry issued uniform and badge. • Travel up to 50% annually to and from centers within the assigned regional territory to support onsite visits (combination of day trips and overnight trips). • Other duties as assigned to support department operations. Minimum Requirements • Must live in the Great Lakes territory • Current CMA/CCMA/RMA Certificate or completion of a CAAHEP accredited/medical assisting program, in lieu of, we will consider candidates with relevant work experience (preferably in primary care: internal medicine, geriatrics, or family medicine) • Reliable transportation and unrestricted driver’s license • Willing to travel per the above requirements. May require occasional weekends as needed but primary work schedule will be M - F. • Proficient in Microsoft Software (Word, Excel, PowerPoint, and Outlook), Virtual Meeting Programs, Various EHR Systems. • May be required to have COVID-19/FLU vaccination per the facilities state guidelines. • Excellent verbal/written communication and presentation skills. • Strong analytical and problem-solving skills • Ability to show strong personal initiative and take ownership over work results. • High attention to detail, accuracy and follow through.
