About the Role: The Domestic Violence Rapid Rehousing Case Manager is a vital team member of the Domestic Violence and Supportive Housing (DVSH) program at Shelter House. The case manager supports survivors of domestic and sexual violence, stalking, and/or human trafficking who are experiencing homelessness as a result of fleeing abuse. This position is responsible for providing direct services by collaborating with survivors to identify and assess needs, facilitate connections to appropriate resources, and support survivors in increasing their safety and housing stability. The case manager uses their knowledge of housing opportunities and services in the community to assist survivors in obtaining and maintaining permanent, safe housing. In addition to support with housing identification, wraparound case management is utilized to provide families with connections to resources to address a variety of other needs such as legal, medical, and financial assistance. Case management services are provided in a client-centered manner driven by the survivor’s self-identified goals and with respect for self-determination. How you will contribute: Collaborate with clients to evaluate safety and social needs, housing history and barriers, financial management and budgeting, and develop and achieve individualized goals Maintain a caseload of between 12-18 clients at a time, providing client-centered, trauma-informed services Provide mobile advocacy services by regularly meeting with clients in a safe location in the community Engage with clients to create individualized safety plans and educate clients on the dynamics of domestic violence Identify resources to help clients increase their levels of physical and emotional safety Assist clients in filing for protective orders and support clients with court accompaniment Collaborate with clients to develop sustainable budgets and facilitate connections to financial literacy programs Utilize the housing-first model to develop focused and individualized service plans Provide housing search assistance to clients by identifying leads, assisting with applications, and attending tours Establish relationships with local properties and landlords and advocate on behalf of clients Educate clients on tenant rights and responsibilities and housing discrimination law Coordinate the provision of monthly rental assistance and track all payments provided Document all communication with clients in detailed case notes; ensure client files and data are regularly updated Ensure services are implemented in accordance with agency philosophy, best practices, grant requirements, and all applicable federal, state, and local regulations Partner with community providers and allied professionals to ensure the delivery of comprehensive, wraparound services Participate in regular trainings, staff meetings, and one-on-one supervision meetings with the Senior Case Manager Proficiently utilize Microsoft Office software as a part of day-to-day work requirements Exemplify the Shelter House core values of (iCARE); Inclusivity, Collaboration, Accountability, Respect, and Empowerment Perform other duties deemed necessary to support the program and agency Required: A bachelor’s degree in social work/human services/related field or commensurate experience 1+ years of human services or direct services experience Strong written and oral communication skills Ability to prioritize competing priorities and make sound judgements Ability to complete tasks while navigating frequent interruptions Ability to deliver creative, resourceful solutions to unique challenges Strong organizational and time management skills Valid Driver’s License and reliable transportation Ability to regularly attend in-person meetings Flexibility to work outside of normal business hours when needed (excluding weekends) Preferred: Experience providing services to survivors of domestic violence and/or people experiencing homelessness Fluency in a language other than English (ex. About the Role: The case manager works under the supervision of the Senior Case Manager and assists low-income working families who have experienced homelessness and those at risk of homelessness. Case manager’s primary responsibilities are to the Family PASS Self-Sufficiency Program, which provides long-term intensive case management for up to three years and works with county resources to help clients attain the education and skills training needed to become economically self-sufficient with a focus on increased earnings through job training and additional education. The case manager supports a caseload of 15-20 households. Case Managers are to implement services with a trauma-informed, strengths-based, and client-centered approach while assisting households to work toward self-sufficiency. The case manager works closely with their supervisor to accomplish program goals and priorities in accordance with agency philosophy and values, and best practices for homelessness and survivors of domestic violence. Duties and Responsibilities: Interviewing and screening clients for eligibility for Family PASS’s Self-Sufficiency Program. Assessment of client strengths and needs. Complete ongoing biopsychosocial-spiritual assessment to determine client strengths and needs and to ensure stability. Develop and/or revises individualized client-centered case plan goals and action plans. Coordination of linkage and referral to community resources to include services such as education/training, career counseling, mental health services, financial literacy, legal services, childcare, medical/dental services, etc. Using a holistic approach consult with supervisors, and/or other programs to help families overcome barriers to meeting goals and objectives. Advocate for client needs to ensure housing and financial stability. Plan, schedule, coordinate, and facilitate meetings with clients on an ongoing and as needed basis. Be prepared for and actively participate in staff meetings, training, and one on one supervisions and any other meetings determined necessary. Creating, tracking, and maintaining client case files in accordance with agency and best practice standards. Timely documentation of client contacts and case management activities in relevant databases and client files. Ensure client data is entered into client lists and other relevant databases in an accurate and timely manner. Perform other duties deemed necessary to support the program and agency.
