Our team prioritizes effective coordination of care to prevent hospitalizations through multi-level communication and real-time assessments. We partner with Post Acute Medical to provide a seamless continuum of care, ensuring smooth transitions for our clients. We closely collaborate with community providers for thorough risk assessments and educate clients and caregivers about personalized care plans to help regain independence at home. Role Description This is a full-time, remote role for a Home Health Nurse RN. The Home Health Nurse RN will be responsible for providing nursing care, medicine administration, and patient care in the home setting. The role includes wound care, conducting thorough risk assessments, and developing personalized care plans. The nurse will work closely with clients and their caregivers, guiding and supporting them throughout the care process to ensure they understand and follow their care plans effectively. Qualifications • Nursing skills, including patient assessments and care planning • Medicine administration skills • Experience in Home Care and Patient Care • Wound Care expertise • Strong communication and interpersonal skills • Ability to work independently and remotely • Registered Nurse license in the state of Texas • Prior experience in home health 1 year minimum • CPR certification • Experience documentation OASIS/SOC Paperwork Required
