Oversee daily operations, manage staff, ensure resident care, and maintain compliance for a geriatric health program. | Bachelor’s degree in social work, nursing, or related field, with 5+ years in program management, geriatrics or social services, and experience managing budgets over $500,000. | Description Job Title: Program Manager Status: Full Time Classification: Exempt Location: Mountain View, CA The Program Manager reports to the Menlo Park Site Director and is responsible for overseeing the daily operations, strategic planning, and management of the Geriatric Health Care for Homeless Veterans (Geriatric HCHV) program. The Program Manager will lead a team of housing/medical professionals, work closely with healthcare providers and community resources, and ensure that all program goals, regulations, and resident needs are met in a timely and effective manner. Job Duties: Program Management: oversee the overall operations of the Geriatric HCHV program, ensuring that services are delivered effectively and in compliance with all of Nation’s Finest’s policy and procedures, as well as all contract regulatory requirements. Team Leadership: hire, supervise, train, develop and support a multidisciplinary team of staff including LVN Case Managers and CNA/Caregivers. Resident Support: ensure that residents receive the appropriate housing services and care tailored to their needs. Oversee the development of care plans and collaborate with internal and external professional care teams including property management company representatives to meet medical, social, housing, and emotional needs of elderly residents. May include transporting participants to appointments. Budget and Resource Management: In coordination with the Site Director, manage program budget, including forecasting and monitoring of expenses. Maintain billing requirements daily and submit all billing monthly. Ensure efficient allocation of resources to meet program goals. Stakeholder Engagement: Build and maintain relationships with external partners, including healthcare providers, community organizations, and governmental agencies. Advocate for residents and ensure they have access to necessary social services and medical care. Compliance and Reporting: Ensure that the program complies with regulations and internal policies. Prepare regular reports for stakeholders and funding agencies. Crisis Management: Address and resolve any urgent issues or challenges such as health emergencies, housing instability, or conflicts among residents, providing swift and effective solutions. Training and Development: identify staff training needs and organize professional development opportunities to ensure that the team is well-equipped to meet the evolving needs of elderly residents. Performs other duties as assigned Educational Requirements Bachelor’s degree in social work, nursing, or related field (required) Masters’ degree in related field (preferred) Qualifications: Current Residential Care Facility for the Elderly (RCFE) certification or ability to obtain RCFE certification within 6 months of hire. Minimum 5 years’ experience in program management, with a focus on geriatrics, housing, or social services. Minimum two years’ experience managing an annual program budget of $500,000+ Minimum two years’ experience supervising staff members, with experience supervising in social services or medical settings preferred. Knowledge of Geriatric Services: In depth understanding of the challenges and best practices related to elderly care Proficient in MS Office applications including Outlook, Excel, SharePoint, and Teams. Proven ability to work independently while providing consistent, high-quality judgment and guidance. Valid Drivers’ License, clean driving record, and proof of vehicle insurance Physical Requirements: Requires the ability to occasionally bend, stoop, reach, lift, and move or carry office supplies and materials weighing 20 pounds or less. Finger dexterity required for computer keyboard operation. Must be able to walk up and down up to 4 flights of stairs several times per day. Work Environment: Office located in Menlo Park, CA. Team-oriented office environment as well as participant residential environment in Mountain View, CA. This is an in-person role and is not remote eligible. Offering: $80,000 - $85,000 annually, depending on qualifications and geographical location. The Residential Case Manager is primarily responsible for the day-to-day activities and safety of the clients and all case management services. This position also covers shifts on holidays, and other occasions as assigned. The Residential Case Manager reports directly to the Housing Coordinator and indirectly to Clinical Coordinator for clinical oversight. Job Duties: • Responsible for maintaining a calm, friendly and cooperative atmosphere within the facility and with the participants to promote a clean, orderly and healthy environment at all times. • Assist Housing Coordinator with conducting intakes and assessments of clients for the residential program. • Document client progress and other significant events in individual client case files according to agency standards and as requested. Promptly and accurately report any issues or concerns regarding clients to direct supervisor or designee. Maintain strictest confidentiality with regard to protecting client's privacy and record keeping. • Manage a caseload of clients and develop an Individual Treatment Plan (ITP) or Individual Service Plan (ISP) , with the client, in accordance to agency standards. Ensure clients have a clear understanding of the goals and action steps outlined in their ITP/ISP and receive support in pursuing those goals. • Provide case management services to program clients, including (but not limited to): crisis intervention, individual and group case management services, and drug testing as directed by policy and required by the program. • Utilizing curriculum and materials available, facilitate group sessions as needed/directed. • Conducts checks of bed area, outside grounds and common rooms to ensure clients are involved in program activities or otherwise productively engaged in ITP/ISP tasks. General supervision of the clients and enforcing program rules. • Participate in staff meetings, house meetings, and the Client Review Team (CRT), as necessary. As a member of the CRT, coordinate with other staff to ensure clients are getting quality, fully integrated services. • Support other Nation's Finest case management programs as instructed and coordinate attendance and progress with other case managers. • Observe, monitor and record the medication of participants. Maintain confidential, accurate and timely records of medications (Not applicable to all case managers, program specific) • Perform other duties and tasks as assigned by the Housing Coordinator or designee as appropriate. Certified Case Manager: 1Under Clinical Supervision, Certified Case Manager is Expected to perform the above duties as well as; A. Perform assessments B. Facilitate clinically appropriate groups C. Maintain certification in active status Required Job Skills: • Ability to work independently, with minimal supervision while maintain an effective working relationship with other staff members. • Be detail oriented, able to set priorities, meet deadlines, maintain a professional attitude and conduct appearance at all times. • Able to set and maintain professional boundaries at all times with clients adhering to agency policy and procedures. • Excellent inter-personal communication skills with the ability to interact with participants of varying ethnic, social or economic backgrounds. Knowledge of mental health issues, drug and alcohol addiction desired. • Experience working with a homeless population highly desired along with a knowledge of veteran related issues. • Current working knowledge of community resources available to assist clients with achieving goals on their ITP. • Experience and knowledge of conducting individual case management and group sessions. • Proficient in office software including: Word, Excel, Outlook and internet use. Qualifications: 2 years' experience in case management and experience in homeless programs preferred. Certified title needs experience facilitating recovery groups and have active certification with state licensing board. AA degree or higher in human services, social work or psychology. Veteran status desirable but not required. Strong ability to create and maintain professional employee-client boundaries. Offering: Full-time positions eligible for comprehensive benefits package. Both full and part-time employees are eligible for generous paid time off and retirement contributions. $24.95 - $30.20 per hour to start. DOE and region.
