We Are Currently Seeking An Experienced HOME CARE MEDICAL SOCIAL WORKER Part Time (Covering Sanford, Siler City & Durham) Job Description • Provides patient care services and documentation of patient care data. • Provides clinical assessment for the development of individualized care plan, and set goals for patient treatment. • Applies a working knowledge of psycho-social principles and family dynamic development. Identification and assessment of social, emotional, financial, and environmental factors which many affect the medical plan of treatment and desired outcome. • Communicate with agency personnel and coordinate with other services in providing optimum care. • Relate well to patient/family at all levels responding professionally to verbal and nonverbal communicators. • Utilize community resource planning in providing patient services, and educate patients/family on how to utilize these resources. Job Requirements • Master’s degree in social work, and a minimum of two years practical experience, preferably in the home health environment. • Strong interpersonal skills, with the ability to effectively communicate within a multidisciplinary setting. • Must accept travel and working conditions assigned. • Minimum of twelve hours of continuing education annually. We are currently seeking an experienced: HOME CARE ADMINISTRATOR Full Time Job Description: • Ensures the efficient, effective utilization of existing staffing resources. • Evaluates and monitors the quality of services delivered and ensure compliance with all applicable regulatory requirements. • Supervises and evaluates job performance of each Home Care Clinical Manager and Home Care Operations Support Technician to ensure compliance with established performance expectations and standards of practice. • Assists the Home Care Clinical Manager with performance management of clinical staff, as indicated. • Participates in orientation, competency evaluation, and education/training activities that are appropriate to identified learning needs of all Home Care staff. • Monitors clinical processes, charting/documentation, and financial data to ensure that all services are provided efficiently and effectively with appropriate resource utilization. • Facilitates the delivery of Home Care services by acting as a resource for staff, physicians, referral sources, patients, and other internal and external customers. • Provides direct patient care to include case management, care planning, and coordination of services, as necessary. • Performs human resources functions in accordance with agency policy and procedure to ensure compliance with applicable regulatory, OSHA, and CDC guidelines, and accepted principles of infection control. • Performs additional duties as assigned. Job Requirements: • Registered nurse with a current NC license or other health-related degrees, including healthcare administration would be considered with the right mix of management experience and skills. • Minimum of five years clinical experience consistent with training/education/degrees. • Minimum of three years of supervisory experience. • Knowledgeable of Home Care operational procedures and clinical processes. • Understanding of the principles of Federal Condition of Participation (CoPs), State and Local regulations, and financial management of home care services. • Ability to analyze financial and statistical data. • Ability to demonstrate high-level problem-solving and reasoning skills. • Good clinical nursing skills or clinical skills commiserate with training/education/degrees. • Current knowledge of Medicare/Medicaid coverage and Home Care documentation guidelines and standards of medical record keeping. • Ability to use a personal computer and have a working knowledge of a variety of computer applications (Microsoft Windows, Word, Excel, Outlook). • Excellent interpersonal skills including the ability to interact and communicate in a professional and tactful manner with all internal and external customers. • CPR certified. • Minimum of 12 hours of continuing education required annually.
