Coordinate doctor appointments, manage medications, review medical files, provide health coaching, and coordinate long-term care benefits. | Must have an RN license, preferably a BSN degree, prior case management experience, English fluency, and computer literacy. | Job description IndeQuest, a leading provider of nurse case management services, is seeking a part-time nurse case manager (with the potential to grow to full time over time) in Grand Rapids, MI. Hours will slowly build to a part-time caseload and, if preferred by the employee, can continue to grow into a full-time case load as more clients sign on to services. Responsibilities: • Attend and coordinate doctor appointments as needed • Coordinate DME, home health care, and therapies as needed • Coordinate LTC benefits as needed • Update medical status with client's family as appropriate • Medication management • Medical file review • Health coaching and education • Provide ALF placement services as needed Qualifications: • RN License is required • College Degree in Nursing; BSN preferred • English as a primary language • Prior case management experience preferred • Computer literacy Pay: $43.00 - $46.00 per hour Benefits: • Flexible schedule Schedule: • Monday to Friday Work Location: In person/telephonic/Grand Rapids area
