Prioritizes and handles heavy work volume and handle routine matters to minimize interruptions and enhance efficiency of assigned executive(s). May handle work on behalf of Board of Directors and related projects. Handles a high level of confidentiality with assigned work. May supervise the work of others. This position requires strong initiative and diplomacy. High level of autonomy. Works under very minimal supervision. Essential Functions Assists executive and relieves assigned executives and staff of designated administrative details. Receives telephone calls, makes transfers and takes messages. Screens, sorts, disposes of and replies to incoming correspondence. Prepares letters, memorandum, statistical reports, summaries, and other materials through verbal dictation, knowledge of organizational policy, or other various sources of data and information. Does so with a high level of confidentiality. Coordinates meetings, facility logistics, prepares presentations, distributes materials and takes minutes. Retains various catalog records. Maintains daily appointment calendars; makes travel and lodging arrangements as required. Provides back up coverage to other clerical staff as necessary. May assist in supervising and coordinating the activities of at least two or more clerical staff support staff. Helps plan, establish and revise work assignments. May help settle employee problems and suggest disciplinary action. Coordinates major meetings including collection and preparation of discussion and meetings with the Board of Directors, executive staff, and outside individuals. Material is prepared within appropriate timeframes and standards of completeness for each setting as demonstrated by customer feedback and documentation. Coordinates and completes special projects in a timely manner. Appropriately prioritizes all tasks so that deadlines are met with continued focus on quality. Keeps staff apprised of any critical or major situation. Prepares and monitors the department administrative budget. Performs other duties as assigned. Education Bachelor's Degree - Preferred Associate's Degree or 2 years' work related experience - Required Experience 5 years experience in the field - Required 5 years plus experience in the field or in related area with at least 1 year lead or supervisory work experience. - Preferred Licenses and Certifications

Responsibilities Job Summary The Manager for Comprehensive Care Coordination program (CCC) is responsible for the continued development and implementation of the CCC Program. The manager is responsible for adhering to the Intensive Outpatient Care Program model (IOCP), operational policies and procedures, and fostering a creative and innovative environment to assist in meeting network strategic initiatives. Serves as a resource to the provider practices and medical staffs. Collaborates with the Manager for TCM program; Practice Managers; Primary Providers; CCC Supervisors, Hospitalists, and other circle of care team members during a patient care coordination activity in the outpatient setting. Supports and contributes to the development and delivery of all CCC educational programs. Essential Functions Manages all CCC operations. This role establishes, and reviews staff productivity, reduction in readmission rates, standardized work in IOCP model of care, quality audits on work processes, outcome monitoring, resource allocations and adjustments when variances occur, coaching and training, and appropriate post discharge TCM/CCM calls. Responsible for quality review oversight of team performance. Addresses employee problems and administers appropriate disciplinary action. Ensures the CCC role is adherent to the IOCP program plan to include scope, achieving goals, roles & responsibilities, measurable objectives, issues, and risk assessments. Ensures CCC programs are efficiently running. Oversee the supervisors and ensure adequate staffing levels and caseloads, inclusive of hiring/onboarding, evaluating, managing performance, coaching and developing employees ensuring employee productivity, quality of work, and alignment of goals with our mission, vision, and values. Ensure compliance with all policies, financial stewardship by being accountable for the financial integrity around development and maintenance of department budget. Monitor consistent entry of program metrics, outcomes monitoring, and reports (especially in the area of prior authorization timelines). Designs program quality monitoring reports and tracks results. Utilizes a variety of reporting tools with multiple data sources to include predictive analytic reports. Monitors daily, weekly, and monthly CCC report and YTD outcomes: including readmission and referral data. Identifies trends associated with denials, Out of Network services, formulary exceptions, readmissions, high risk patients and preferred provider referral patterns. Collaborates with PPLs, IT, and other staff in the program design. Develops, arranges and documents educational programs. Performs community outreach with organization preferred providers. Builds a repository of community resource tools. Keeps current on HIPAA, CMS, Federal, State, and Local regulatory requirements, new project management strategies and techniques. Maintains regulatory surveillance on intra-rater reliability testing annually. Adheres to all medical management regulations and timeframes for payors and members. Performs other duties as assigned. Responsibilities Job Summary Under the direction of the IT manager, the Epic Client Systems Administrator (ECSA) handles all daily operations of the Epic application system components. Manages service desk tickets as well as work requests for the support of Epic application infrastructure. Ensure that the health of the Epic Infrastructure is properly maintained through the following activities: troubleshooting system issues; upgrades and patches for system stability; tuning for system performance and properly configuring different components of the Epic Infrastructure. In addition, the Epic Client System Administrator will provide guidance, mentoring, training and participate in an on-call rotation that includes facilitating off-hour work functions in support of incidents, upgrades, patches, and project task fulfillment. • Participates in the deployment, configuration, maintenance, and troubleshooting of EMR infrastructure: Mentors and responds to requests for level I and II incident support. Manages tickets for incidents and requests. Performs proactive Epic application infrastructure problem detection, validates application functionality, and develops training materials for customers and staff. Documents & submits new content to the knowledgebase, maintains team wiki, creates process maps, develops new framework & processes around problem management. Coaches and trains other teams that are less experienced, performs quality assurance on incidents and requests to identify coaching opportunities. Uses communication skills to listen to, identify & address customer needs while building a relationship and rapport with customers. Prioritizes and responds promptly based on severity levels to needs of the customer. Proactively or timely recognizes opportunities for service recovery and acts to resolve issues. • Training (lead by senior peer or self-guided) and completing Epic certifications • Performs other duties as assigned.
