To demonstrate support of the hospital mission and vision by providing nursing, case management/disease management services to clients and families to help in the coordination and management of individuals’ health needs through the full continuum. The Registered Nurse demonstrates the ability to assess, coordinate, plan, monitor, implement and evaluate the patient’s plan of care while ensuring and facilitating the achievement of optimal quality, clinical and financial outcomes. Performs assessment of clinical and life issues for the patients of COH. The successful candidate: Must be able to work FT 7a-7p and/or 8a-8p Mon-Friday, and Sundays. Weekends and holidays are required as needed - 2 Sundays per 6 weeks. Provides disease management and, understanding oncology, chemotherapy and works with patients and families throughout the continuum of care to provide an understanding of their the side effects, is able to monitor and address symptoms. Works with patients and families throughout the continuum of care to provide an understanding of their disease, their treatment plans, and address patient quality of life issues. Document assessment, toxicity screen, and interventions entered in a timely manner in the electronic medical record. Collaborates with, and supports, provider team with daily assistance in managing and coordinating patient care. Evaluates and assesses health issues presented by patients and their caregivers and communicates pertinent and necessary issues to the physician. Effectively and efficiently triages patient medical issues either in person or by telephone. Intervenes appropriately with use of orders and/or consultation with the interdisciplinary health care team. Provides professional nursing services to address patient’s health and other related issues in cooperation with other clinical and clerical team members. Discusses and educates patients and families. Documents timely and thoroughly in the electronic medical record. Works as a part of the team as a resource to facilitate communication and services between physicians, patients, families and other healthcare team members including external health care agencies, to facilitate the plan of care, including home services and medical equipment. Qualifications Your qualifications should include: Current state Registered Nurse License required Bachelor of Science Degree required or enrolled within 6 months of hire and completed within 3 years from hire date BLS & ACLS certification required. Plans, delivers, directs, coordinates and evaluates therapeutic interventions based on needs identified in the assessment process, and in cooperation with other clinical and clerical team members. Documents interventions provided. Provides patient/family education (in regard to disease process, side effects of treatment and health promotion) as well as support and reassurance, ensuring an informed decision is being made regarding the individuals therapeutic plan and documents according to department standard. Ensures compliance with hospital policies and accrediting agencies and according to best nursing practice. Participates in the development, review and updating of policies and procedures. The successful candidate: Position Summary: Design, build, and test an assigned Epic module. Maintain documentation of business and system configuration requirements. Configure the Epic software according to decisions made by SMEs such as templates, drop-down boxes and pick lists. Complete assigned team tasks as documented in the work plan. Report status of task completion to assigned Team Lead. Provide timely alerts about endangered milestones and deliverables. RESPONSIBILITIES: • Analyzes, develops, configures, tests and implements Electronic Health Record (EHR) software and related analytics on time with minimal supervision. • Performs system configuration functions using on-line architectural tools with minimal supervision. Prepares detailed program specifications and flowcharts, and coordinates the system's installation with the user department. • Provides status reports to management when required. • Attends meetings as required and participates on committees as directed. • Prepares charts and diagrams to assist in problem analysis and submits recommendations for solutions. • Acts as user liaison between clinical or business departments and Information Systems; providing domain expertise and experience for multiple interacting healthcare departments. • Works closely with operational stakeholders to properly engineer workflow. • Leads others to adhere to established industry best practices, standards, and quality. • Maintains a positive image when dealing with department personnel and other City of Hope employees.
