The Case Manager acts to ensure the stability of individuals within the program properties and will be familiar with and able to access resources for clients, including alcohol and drug treatment, mental health treatment and any other identified needs. The Case Manager must have a good understanding of dual relationships, professional boundaries, and appropriate behavior. The candidate will have the skills needed to support and assist clients in improving and maintaining recovery, retaining housing, and achieving self-sufficiency. Location: Hazel Heights (12621 SE Stark Ave. Portland, OR 97233) Schedule: Monday - Friday, 9:00am - 5:30pm Compensation: $21.19/hr - $27.55/hr Minimum Qualifications • High School Diploma or GED and six months of experience working with the homeless population, behavioral health environment, human services, or related environment; OR 6 months experience in a high-volume customer service role. • Must pass a pre-employment drug screen and background check. • Will be required to carry an agency cell phone for work use. Cell phones will be provided by Central City Concern. • Must adhere to agency’s non-discrimination policies. • Ability to adhere to Central City Concern’s drug-free workplace which encourages a safe, healthy, and productive work environment and complies with the Drug-Free Workplace Ace of 1988. Preferred Qualifications • Valid driver's license preferred. If required to drive, must pass a DMV background check, and be designated an "acceptable" driver as outlined in Central City Concern's Fleet Safety Policy. If required to drive, must pass an initial driver's training within 60 days of being an approved driver and continued recertification training. • Understanding of recovery terminology and ability to apply the concepts highly preferred. • Bi-lingual, Spanish-speaking preferred. Essential Duties And Responsibilities • Work closely with the Supportive Housing and Health Services team ensuring a safe and healthy recovering environment. • Build understanding of and appreciation for diversity among residents and support their connection to culturally responsive and culturally specific services as requested. • Process resident/client intakes and discharges with completion of associated forms. • Maintain resident/client files including progress notes and other records. • Meet bi-weekly with clients along with other referral sources as needed. • Refer clients to Employment Access Center along with other CCC (Central City Concern) and outside agencies as appropriate. • Transport clients to the housing and medical appointments and picks up food boxes. • Identify community and individual issues which could result in a change in a residents/client’s status and work to achieve appropriate resolution. • Connect residents/clients to appropriate self-sufficiency and community-based resources. • Network with various community providers. • Attend appropriate community professional meetings. Assist in completing required reports for the Housing Department, Supportive Housing Department, Health Services, and/or Administration Department promptly and accurately. • Enter data into the Homeless Management Information System. • Monitor office and answer telephone as necessary. • Participate in compliance, quality assurance and quality improvement activities as directed. • Adhere to all state and federal privacy and security regulations applicable to the program, and to CCC policies and agreements regarding confidentiality, privacy, and security. This includes immediately reporting any breach of protected health information or personal identification information of any person receiving CCC services by CCC or an outside provider to the CCC Compliance Department, as well as to your supervisor or their designee. • Perform other duties as assigned Skills And Abilities • Ability to consider the impacts and outcomes for underserved communities during decision-making processes. • Ability to consider impacts of systems of oppression, structural racism, and individual bias on client outcomes. • Knowledge of chemical dependency as a disease, treatment, and intervention methods. • Ability to understand mental health assessment, treatment and service terminology and apply the concepts. • Identify resident issues and support them through information sharing and educating them about building rules and policies. • Follow up regularly with residents to ensure they are getting their needs met. • Interview all applicants prior to move-in to complete basic needs assessment, ensure they meet building criteria. • Refer to employment opportunities, benefits and entitlements, medical and mental health resources as needed or desired by residents, • Respond to and investigate resident reports of building, community, CCC process and procedures and CCC employee issues that arise related to the building. • Create and execute internal community programs within the building in partnership with the services team. • Ability to work as a team member and to establish priorities. • Ability to display a non-judgmental attitude, and a willingness to learn new skills. • Demonstrated ability to work without supervision. • Demonstrated knowledge of community, social agencies, and self-help groups. • Sufficient manual dexterity and physical ability to perform assigned tasks. • Ability to communicate clearly and concisely both verbally and in writing. • Ability to manage time and meet deadlines. • Ability to understand and follow verbal and written instructions. • Ability to work courteously with the public, tenants, and co-workers. • Ability to maintain accurate records and necessary paperwork. • Ability to access and enter data into a web-based data system. • Ability to learn and apply training instruction. • Knowledge of de-escalation methods or ability to be trained in de-escalation methods. • Ability to effectively communicate with clients, co-workers, corrections personnel, police, merchants, the public at large and supervisor. • Ability to provide leadership. • Ability to provide advocacy, as appropriate. • Professional maintenance of appropriate boundaries. • Ability to establish and maintain cooperative working relationships with those contacted during work. The Blackburn Center is an innovative program that will offer integrated housing, health care, and employment services with the long term goal of eliminating homelessness. We seek to change how these services are delivered to all people, especially vulnerable populations. This position will work as a member of a multi-disciplinary team at the Blackburn Center to deliver comprehensive, person-centered, integrated services to the Portland Eastside community. The Blackburn Center Environmental Services Associate is responsible for the janitorial services of the Blackburn Center, an integrated health services and housing facility. This position is an integral member of the facilities team and is an integral member of the facilities team and reports directly to the Blackburn Facilities Operations Manager (FOM) for daily operational matters, performance and services oversight, as well as matters requiring collaboration, optimization, scheduling and coordination. Schedule: Monday -Friday 10:00am - 6:30pm Location: Blackburn Center, 12121 E. Daily Services include vacuuming high traffic areas, sweeping, dust mopping, wet mopping emptying trach cans and replacing liners, spot cleaning stains and spills (including bio-hazard) on all surfaces. Weekly services include detailed wall-to-wall vacuuming, spot cleaning walls and cleaning glass surfaces and doors. Monthly services include detailed high and low dusting and annual services include furniture cleaning twice per year. Schedule of services are typical and subject to change. • Perform cleaning services for restrooms. Daily services include sweeping, dust mopping, wet mopping emptying trach cans and replacing liners, spot cleaning stains and spills on all surfaces (including bio-hazard). Daily services include vacuuming high traffic areas, sweeping, dust mopping, wet mopping, emptying trach cans and replacing liners, spot cleaning stains and spills on all surfaces (including bio-hazard). Weekly services include detailed low dusting, detailed wall-to-wall vacuuming, damp mopping of hard surface floors, spot cleaning walls and cleaning glass surfaces and doors. Monthly services include detailed high dusting annual services include quarterly dusting of blinds and window sill and furniture cleaning twice per year. Schedule of services are typical and subject to change. • Perform cleaning services for exam rooms, clinical and lab space and pharmacy. Daily services include vacuuming high traffic areas, detailed wall-to-wall vacuuming, sweeping, dust mopping, wet mopping, emptying trach cans and replacing liners, clean and sanitize horizontal surfaces including exam tables, carts, countertops, toilets and sinks, clean furniture, check and fill glove, soap and towel dispensers, spot cleaning stains and spills on all surfaces (including bio-hazard) and clean monitors, lights, computer towers and beds. Weekly services include detailed low dusting, spot cleaning walls and cleaning glass surfaces and doors. Monthly services include detailed high dusting annual services include quarterly dusting of blinds and window sill and furniture cleaning twice per year. Schedule of services are typical and subject to change. • Assist with hard and/or deep cleaning as and when directed. Service may include stripping and waxing of hard surface floors, spray buffing of tile and stone floor surfaces and deep carpet cleaning. • Clean sidewalks and other outside public areas as and when assigned. • Clean sidewalks, pressure wash exterior surfaces and other public areas of the clinical and office portions for Blackburn Center as and when assigned. • Maintain work areas in a safe, clean, and orderly manner. • Adhere to all standards and specifications for clinical janitorial needs. • Complete all necessary paperwork as and when assigned and necessary to complete job duties. • Attend all required meetings. • Adhere to safety policies and procedures. • Support other functions of Blackburn Center as and when assigned. • Provide support to all facilities and environmental services within the BBC care continuum, including unit-turns, as requested. • Work as a member of the Blackburn Center multi-disciplinary team to deliver comprehensive, client centered and trauma informed integrated services. • Build understanding of and appreciation for diversity among Blackburn Center clients and support their connection to culturally responsive and culturally specific services as requested. • Adhere to all state and federal privacy regulations, including HIPAA and 42 CFR Part 2, and to CCC policies and agreements regarding confidentiality, privacy, and security.
