Now we’re looking to hire a Clinical Care Coordinator. The Clinical Care Coordinator is a cross-functional leader driving seamless, high-quality care across clinical, care navigation, and business development teams. This role blends direct client support and referral management, overseeing the outbound referral and aftercare planning process for all patients. This role centers on delivering comprehensive case management services. What You'll Do Connect clients and families to appropriate clinical resources (coaches, support groups, additional services) in coordination with the clinical team and business development Serve as system navigator and point-of-contact for clients and families during transfer and discharge process and lead related care coordination efforts through relationships with treatment facilities. Provide advanced case management and social work support to clients and families, including navigations of FMLA, unemployment, school accommodations, and other services Coordinate with business development to strategically foster relationships within the treatment community Communicate with clients through face-to-face meetings and asynchronous messaging. Leverage Salesforce to manage referrals and maintain high-value provider relationships. Create and maintain a robust spreadsheet of referrals Document all relevant information in the client’s medical record in a timely manner. Participate in multi-disciplinary team meetings. Assure each patient’s discharge summary is provided to all members of the outpatient team Represent Within Health at external events and business development initiatives. Lead all aspects of the outbound referral process, ensuring timely transitions across levels of care. Other related duties as assigned based on need and availability. Who You Are Bachelor’s degree or higher education 2 year(s) of experience with eating disorder clients and families preferred. Prior case management experience, preferred. Strong attention to detail. Ability to treat clients with dignity and empathy. Strong listening and communication skills. Acute observation skills. Computer literacy. Willingness to treat clients with Eating Disorders and Disordered Eating. Understanding of diverse cultures and gender-specific issues and ability to incorporate needs of gender and culturally diverse groups into practice settings. Excellent organizational and time management skills. Ability to prioritize workload and work independently.
