Full Description

Position Summary The Social Services Coordinator assists with resolving social issues, providing resources for residents and performing administrative duties in accordance with their scope of practice and state and federal social work guidelines. Duties and Responsibilities 1. Reviews each potential resident for admission and provides PAS information in conforming to state-mandated regulations. 2. Develops an initial care plan based on the physical and psychological needs of the resident. 3. Continuously updates resident care plans and documentation regarding resident condition. 4. Provides direct counseling to residents, family members, and/or resident representatives. 5. Works closely with the healthcare team to identify, assess, and address any needs that can be met either through social services, a chaplain, or a mental health provider. 6. Assists the resident in adjusting to the community and promotes continuity of relationships with family and community. 7. Coordinates relocation planning, including advising and referring to community resources before and during relocation. Coordinates transfers of residents from the residential building to the health center. 8. Establishes, advises, and implements policies to ensure the safeguarding of confidential information obtained in the compilation of residents' social histories and assumes privacy of interviews. 9. Participates in the quality assurance process as assigned by the director of nursing. 10. Knows the RAI process and performs MDS assessments as needed. 11. Educates staff regarding the psychosocial and behavioral needs of residents. 12. Performs other duties as assigned. Job Qualifications Required: a high school diploma or GED equivalent and the successful completion of the 20-hour social service course or other social worker designee program within the state. Note: a person who has not completed a social service course approved by the council shall receive consultation from a person who meets the qualifications of (a), (b), or (c) above until he/she has satisfactorily completed the council-approved course. Preferred: bachelor's or master's degree in social work or gerontology or an ordained minister, priest, rabbi, or sister/brother of a religious institution or psychologist.
