Job DetailsJob Location: Stockton - 2291 W March - Stockton, CA 95207Position Type: Full TimeEducation Level: High SchoolSalary Range: $27.00 - $27.00 HourlyJob Category: Nonprofit - Social ServicesPOSITION SUMMARY: Person in this position will be responsible providing direct support to homeless veterans by assessing their needs, developing individualized service plans, and connecting them with resources to achieve employment and long-term stability. This role involves close collaboration with program participants, employment specialists, and community partners to address barriers to employment and promote successful reintegration into the workforce. ESSENTIAL JOB FUNCTIONS: Conduct comprehensive assessments to identify veterans’ needs, strengths, and barriers to employment. Develop and implement individualized service plans, coordinating employment and supportive services. Provide case management support, including referrals for housing, mental health services, substance use treatment, transportation, and other essential resources. Work closely with the Employment Specialist to align job placement efforts with participants’ skills and career goals. Monitor and document participant progress, ensuring compliance with HVRP grant requirements. Advocate for veterans and assist in navigating systems such as VA benefits, workforce development programs, and community services. Facilitate life skills workshops and employment readiness training. Maintain accurate case notes and data tracking for program reporting and performance evaluation. Must Embrace and embody the mission, vision, guiding principles, clinical vision, and goals of WestCare Foundation; and Perform all other duties as assigned. QualificationsESSENTIAL QUALIFICATIONS: To perform this job successfully, an individual must be able to perform the duties and responsibilities satisfactorily. The requirements listed below are representative of the knowledge, skill, and/or ability required. Experience and Competencies: Knowledge of community resources, veteran benefits, and employment support programs. Strong problem-solving and crisis intervention skills. Experience working with homeless populations, individuals with substance use disorders, or mental health challenges is a plus. Proficiency in case documentation and program reporting. Commitment to supporting veterans in overcoming challenges and achieving self-sufficiency. Adherence to the highest standard of ethical conduct, especially to standards governing confidentiality. Professional appearance and demeanor. Military/Veteran preferred. Must have the ability to obtain and maintain a successful background check. Valid driver’s license and ability to obtain and maintain WestCare’s vehicle insurance is required. CPR Certification, First Aid Certification, and an annual Tuberculosis test. During your tenure with WestCare there may be new requirements, including, but not limited to vaccinations that are issued by local, State, Federal, and/or Funders that WestCare may have to comply with. Should this occur Human Resources or appropriate personnel will inform you. EDUCATION and/or EXPERIENCE: Minimum 2 years of experience in case management, workforce development, or veteran services. Graduation from an accredited four-year college with a degree in Social Work, Human Services, Business Administration, or related field is preferred. High School diploma or its equivalent is required. LICENSURE/CERTIFICATION: Not applicable for this position. WORKING CONDITIONS: Work is primarily performed in an office or in an office setting. Some outdoor activities may be required.
