JOB SUMMARY: The Home Care Scheduler is responsible for scheduling home care for PACE participants. This includes scheduling internal home care staff and working with outside contracted vendors. Other duties include participation in training and scheduling of Home Health Aides - CNA and Personal Care Staff. The Home Care Scheduler coordinates and maintains the schedule for home care visits. Also responsible for providing a variety of office support functions including but not limited to: scheduling and coordinating office visits, making and receiving telephone calls, filing, and data entry. The position provides administrative support to the clinic staff, providers, therapies departments and external contracted providers while ensuring all participant appointments are scheduled, tracked and reported to all relevant stakeholders. ESSENTIAL DUTIES AND RESPONSIBILITIES: • Creates and maintains the home care schedules • Assesses participant needs for Personal Alert Response Systems, initiates the referral to contracted provider and ensures timely set up in coordination with the IDT • Meets with participants to discuss any home care service or care concerns • Meets the needs of participants through scheduling design and creative solutions • Monitors documentation to ensure home care plans and tasks are completed according to the participant’s plan of care. • Has oversight on expenditures for home care services and supplies • Keeps records of supply inventories for Home Care and completes orders • Coordinates medication and supply deliveries to participants with the IDT • Completes all other job assignments as requested by the Home Care Coordinator - RN KNOWLEDGE, SKILLS AND ABILITIES: • Technical Capacity • Communication Proficiency • Thoroughness • Customer Service/Participant Focus • Critical Evaluation • Ethical Practice • Flexibility • Relationship Management • Stress Management/Composure • Demonstrates support of the Company’s Mission, Vision and Core Values • Provides Exceptional Customer Service • Ensures discretion with confidential information in accordance with HIPAA guidelines • Supports a collaborative work environment including courteous, helpful and professional behavior • Embraces Organizational Excellence through practicing individual time management, efficiency and effectiveness and participating in continuous improvement efforts • Adheres to and supports all Company policies and procedures • Supports and practices safe work habits in accordance with policies and procedures • Brings ideas, problems and concerns forward and participates in resolution and implementation • Participates in and completes regulatory compliance trainings within the prescribed deadlines • Attends required meetings • Maintains skills and knowledge required including written and verbal communication, best practices for industry standards, and computer competency • Electronic medical records experience is preferred • Knowledge of medical terminology is preferred • Knowledge of standard medical office practices and procedures including filing, record keeping, and the use of standard office equipment such as computer and copy reproduction equipment • Computer proficiency in office software and procedures with ability to maintain accurate records, compile data and prepare reports. • Good organizational skills with the ability to be flexible and demonstrate initiative MINIMUM QUALIFICATIONS AND REQUIREMENTS: • High school diploma or GED. Associate’s degree is preferred. • 2 or more years of healthcare scheduling experience is preferred • Must have a current Maryland driver’s license and driving record. Must have reliable transportation. Must be able to communicate effectively in verbal and written form with all levels of personnel within and outside of the organization. • Pressure Factor - Requires working under stressful conditions. Moderate pressure to meet scheduled and recurring deadlines.
