Role Description This is a part-time remote role for a Care Coordinator focused on CCM and RPM. The Care Coordinator will be responsible for assisting with patient communication, scheduling follow-ups, and supporting care plans. Daily tasks may include coordinating care between patients and healthcare providers, ensuring compliance with care protocols, providing outstanding customer support, and effectively managing patient records. Maintaining a clear line of communication with healthcare teams and patients will be a key aspect of this role. Qualifications • Proficiency in Care Coordination and Case Management • Strong Communication and Customer Service skills • Familiarity with Home Care processes and practices • Attention to detail and ability to handle sensitive medical records responsibly • Ability to work collaboratively with healthcare teams and independently in a remote environment • Prior experience with Chronic Care Management (CCM) and Remote Patient Monitoring (RPM) and complete understanding of their protocols is a must • Basic proficiency in healthcare software or willingness to learn new technologies • An associate degree or higher in a healthcare-related field preferred
