Provide professional nursing care remotely and onsite, coordinate care planning, and support various home-based patient care programs.

Schedule Full Time: 40 hours per week; more than 50% remote General Summary Provides professional nursing care at the intermediate or expert level in accordance with the principles of patient and family-centered care based on the nursing process and according to the clinical standards of the hospital. Coordinates care planning with underdisciplined. May requires specialized skills depending on assignment. This position will support the WellSpan at Home program with the hours divided equally among the Remote Patient Monitoring and Hospital at Home program groups.
