#RN RN Case Manager 36hrs weekly pay $2600 Location: Multiple locations in MA Level 2 trauma Nurse Case Manager needed for acute care hospital to do hospital transition of care discharge planning. Hospital case management and discharge planning. Profiles must detail/be clear in regards to discharge planning Internal floating within the department.
