Adult HIV Program Medical Case Manager, Registered Nurse - Department of Internal Medicine Wayne State University is searching for an experienced Adult HIV Program Medical Case Manager, Registered Nurse - Department of Internal Medicine at its Detroit campus location. Essential functions (job duties): Job Purpose: The Adult HIV Program Medical Case Manager, Registered Nurse provides both direct outpatient clinical care and programmatic leadership in the areas of quality improvement, retention in care and patient outcomes for the Ryan White HIV/AIDS Program. This dual role combines nursing practice within an interdisciplinary HIV care team with responsibility for developing, implementing, and evaluating programmatic activities that improve clinical performance and client engagement. This position ensures that care delivery aligns with federal Ryan White program standards, Health Resources and Services Administration (HRSA) guidance and clinic quality management priorities. The position reports to the Nursing Team Supervisor/Lead Nurse Practitioner. Essential Functions: Provide comprehensive nursing care to people with HIV in an outpatient setting, including medication management, adherence counseling, symptom management, and patient education. Conduct clinical assessments and coordinate care with providers, behavioral health clinicians, pharmacists, and patient advocates to address medical and psychosocial needs. Monitor and report adverse medication events or clinical concerns per established protocols. Facilitate linkage to care for newly diagnosed or re-engaged clients and coordinate laboratory testing and medication initiation. Participate in daily clinic huddles and interdisciplinary case conferences to promote integrated care and continuity across disciplines Develop, document, and implement evidence-based workflows and best practices that enhance patient engagement, adherence, and service coordination. Provide nursing and programmatic input to the development of policies, procedures and clinical protocols that align with Ryan White program expectations. Participate in quarterly CQM Committee meetings, program evaluations, and site visits from HRSA and other funders as a representative of nursing and case management. Contribute to grant reporting, workplan updates, and performance narratives, ensuring accurate reflection of nursing and case management contributions to program outcomes. Perform other related duties as assigned. Unique duties: Qualifications: Education: Bachelor's degree Bachelor’s degree from an accredited college or university in nursing (BSN) preferred. Current Michigan Registered Nurse (RN) license in good standing. An equivalent combination of job-related experience and education will be considered. Experience: Experienced (minimum 2 years of job-related experience) Experience in outpatient/ambulatory clinic settings preferred. Demonstrated experience in quality improvement, data-driven performance monitoring and patient pretention strategies preferred. Knowledge, Skills, And Abilities: Understanding of HIV pathophysiology, antiretroviral therapy, and Ryan White HIV/AIDS Program requirements. Proficiency with electronic medical records (e.g., AthenaHealth, Cerner) and CAREWare data systems. Ability to analyze clinical and programmatic data and translate findings into actionable interventions. Excellent communication and collaboration skills with patients and interdisciplinary staff. Commitment to trauma-informed, culturally competent, and stigma-free care.
