About the Program Health2Home is a medical respite program designed for individuals experiencing homelessness or housing instability following a hospitalization or medical event. This is a safe, supportive environment where participants can recover while receiving coordinated medical and social services. As the Clinical Program Manager, you will play a vital role in client recovery—focusing on stabilization, care planning, and holistic support rather than high-volume bedside care. Key Responsibilities Clinical Leadership • Perform comprehensive nursing assessments • Develop and revise individualized care plans • Monitor participant progress and respond to changes in condition • Delegate tasks to LPNs or Medical Assistants as appropriate • Serve as the primary clinical resource for the program • Coordinate with hospitals, providers, and external clinical partners Program & Team Oversight • Oversee daily program operations and workflow • Supervise Resident Specialists and clinical support staff • Ensure accurate documentation and regulatory compliance • Facilitate team meetings and case reviews • Support continuous improvements in clinical quality and participant outcomes Qualifications Required • Active Michigan Registered Nurse (RN) license (Associates or Bachelor’s degree accepted) • Minimum 3 years of experience in community health, behavioral health, complex care, case management, medical-surgical, or similar areas • Experience providing clinical judgment in dynamic or nontraditional settings • Strong communication and crisis-response skills • Ability to work collaboratively with interdisciplinary teams Preferred • Psychiatric or behavioral health nursing experience • Community health, care coordination, or medical respite experience • Supervisory or team leadership experience • Master’s degree in Nursing, Public Health, Social Work, or related field Why Nurses Choose VOAMI Competitive Benefits • Medical, dental, and vision coverage • Generous paid time off + floating holidays • 403(b) with employer match • Employer-paid life and disability insurance This is nursing with purpose, balance, and impact.
