A company is looking for a Telehealth Physician (Full or Part-time, No On Call, M-F). Key Responsibilities Provide routine telehealth visits with Medicare patients (E / M and Chronic Care Management) Create and manage personalized care plans for ongoing conditions Collaborate with the integrated care team and ensure thorough documentation in EHR Required Qualifications MD or DO, Board Certified / Eligible in Family Medicine, Internal Medicine, or Geriatrics Active medical license; multi-state licensure is a plus Credentialed with major plans : UHC, Aetna, Humana Strong interest in preventive care, senior health, or chronic condition management Tech-savvy and comfortable practicing via telehealth
