A company is looking for a Transitions of Care RN Manager to lead a team of RN Case Managers in delivering exceptional care and improving patient outcomes. Key Responsibilities Coach and support RN Case Managers through various methods including meetings and shadowing Identify workflow challenges and contribute to standardizing best practices across teams Develop onboarding and ongoing education experiences for staff while collaborating with cross-functional leaders Required Qualifications Active RN license (compact / multi-state licensure preferred) Minimum 2 years of direct patient care experience in care management or transitions of care Minimum 2 years of people management experience with a focus on team development Experience managing licensed care management teams is preferred Proven ability to drive clinical and operational outcomes through new initiatives
