A company is looking for a Chronic Care Management Specialist - Clinical Check. Key Responsibilities Ensure the privacy and accuracy of patient information in compliance with HIPAA regulations Conduct thorough reviews of patients' health backgrounds to identify contraindications for rehabilitation programs Input and maintain accurate patient data in medical records systems Required Qualifications Bachelor's degree in Clinical Exercise Physiology or Exercise Science Minimum of two years of experience in a medical office setting, preferably in cardiac and pulmonary rehab Proficiency with medical records systems and software Knowledge of medical terminology and record documentation Familiarity with HIPAA regulations and commitment to patient privacy
