Salary: Temporary RN (1099 Contract) Louisiana Based | 26 Weeks | Remote Work Company: Vigilance Health Location: Remote (Must have an RN licensed in Louisiana or a compact RN license) Type: 1099 Contractor Duration: Short-term (2 to 6 weeks) Start Date: ASAP About the Role: Vigilance Health is seeking a quick, detail-oriented, and licensed Registered Nurse (RN) in Louisiana for a short-term contract (26 weeks) to support our care management program. This is a remote role, perfect for a skilled RN looking to contribute to an innovative population health team while working from the comfort of their home. You will primarily assist in the creation and sign-off of patient care plans as part of our advanced care management services, including support for APCM (Advanced Primary Care Management) and CCM (Chronic Care Management) programs. Youll play a key part in ensuring high-quality care coordination and documentation during a critical project ramp-up phase. What Youll Do: • Support our Care Management team by reviewing, creating, and signing off on comprehensive patient care plans. • Collaborate with health coaches, LVNs, and care managers to ensure care plans meet clinical and regulatory requirements. • Ensure timeliness and accuracy of documentation within the ThoroughCare and/or EHR systems. • Serve as a clinical point of contact for escalated questions related to nursing scope of work during the project. • Provide quality assurance support on completed documentation. • Contribute to a smooth workflow for rapid project deployment across our FQHC partners.
