Vick Street Manor is seeking caring, responsible, and a qualified Personal Care Coordinator to join our assisted living facility in Port Charlotte, Florida. Family Extended Care (Vick Street Manor) is a not-for-profit organization delivering compassionate care to our residents. The ideal candidate will possess a strong background in medical administrative support, with excellent organizational skills and a genuine commitment to patient-centered care. Responsibilities • Act as a role model for shifts supervisor and resident care staff. • Supervises, provides guidance and evaluate all resident care staff in order to provide quality services to all individuals we serve on a daily basis at the direction of the Wellness Coordinator. • Review ADL books daily to assure that the resident care staff is completing their duties and make necessary changes to care plans. • Will be in charge of making sure that monthly summaries are complete for each resident each month. • Assist resident care staff with coordinating daily care for residents and make sure resident care needs are met by resident care staff in feeding, bathing, toileting, dressing, grooming, and incontinence needs. • Make sure that Meds are current in overstock cabinet and conduct a cart audit at the beginning of each month and review the findings with the Wellness Coordinator and Administrator. • Make sure the resident care staff reports on an ongoing basis as to what is happening on their shift (problems, concerns, refusals, etc.) • Prioritize resident needs and work as a team with the resident care aides and other department staff members. • Assist resident care staff with day to day problems. • Transport residents to medical appointments and other destinations at the discretion of the Administrator. • Delegate responsibilities to resident care staff to comply with ALF standards at the direction of the Wellness Coordinator. • Assure that any medical concerns are communicated to the Wellness Coordinator and the Administrator as necessary and follow up these communications by charting the communication. • During emergencies, determine action necessary and initiate 911 procedures when applicable. • Assist the Wellness Coordinator with Doctors orders, charting, rotation of meds as needed and overseeing med-carts and med-techs. • Assist Wellness Coordinator when VA meds need to be ordered and order as needed if Wellness Coordinator is unable. • Ensure that all drug items are in date, properly labeled, controlled and stored in a sealed container and that all medication carts are stocked, secured and maintained. • Report any resident change of medical condition to the Wellness Coordinator or Administrator immediately. • Produce accurate and current documentation relative to residents, i.e. incident report, special requests, behavior and special needs. • Oversee work schedules and staffing to ensure sufficient staff coverage and cover call offs when necessary. • Take call offs. • Function as a resident aide or Med-tech as needed. • Assist in training/orienting all new staff on their shift. • Oversee residents care staff to make sure they are assisting in the dining room meal service. (This includes helping to serve, clearing plates so dessert can be served and observing residents to make sure there are no emergencies during meal time). • Meet with new residents (within hours after admission). Make an initial admission note in the residents chart. Make sure inventory list is complete and put together resident charts. • Assist in ensuring all staff on the floor have appropriate equipment to properly carry out their duties. • Be available via phone at all times for consultation on non medical issues after normal work hours to ensure proper services are provided to facility and individuals we serve. • Participate in resident care and staff meetings, routine and required in-service training and activities as required an as needed. • Be available via phone at all times for consultation on non-medical issues after normal work hours to ensure proper services are provided to facility and individuals we serve. • Participate in resident care and staff meetings, routine and required in-service training and activities as required and as needed. • Be available to facility administrator, if necessary, during times of natural disaster (i.e. hurricane, tornado, fire, etc.) • Must be friendly, courteous, kind and respectful in dealing with residents, families and visitors at all times. • Must treat resident information as confidential information. • Perform other related duties as needed or requested by the administrator. • Participate in "Manager on Duty" rotation when assigned. Requirements • Prior experience working in a medical office setting or healthcare environment is preferred. • Proven ability to provide comprehensive medical administrative support efficiently. • Office experience demonstrating organizational skills and attention to detail. • Excellent interpersonal skills with the ability to communicate clearly and compassionately with diverse populations. • Ability to multitask effectively in a fast-paced environment while maintaining accuracy. • Knowledge of privacy regulations such as HIPAA is desirable but not mandatory; training can be provided.
