WORK SCHEDULE 100% FTE, 40 hours/weekly Day shift, Monday - Friday DEPARTMENT DESCRIPTION The Ambulatory Care Population Health department provides comprehensive nursing care management for high-risk patients across all Harborview primary care clinics. Our team collaborates across the care continuum to ensure safe and effective transitions between care settings, including inpatient units, the emergency department, and specialty clinics. We serve patients with complex medical conditions, as well as significant social and behavioral health needs, striving to deliver coordinated, patient-centered care. Approximately 70% of the RNCM’s time will focus on care management and 30% on serving as an expert diabetes and care management resource across clinics for the management of complex patients. This blended role strengthens transitions of care and enhances coordination not only across inpatient and outpatient settings but also between primary and specialty care—ensuring continuity, reducing fragmentation, and improving outcomes for patients with complex needs. POSITION HIGHLIGHTS Conveniently located near public transportation Make a difference for the patients and employees we serve Opportunity to provide career growth PRIMARY JOB RESPONSIBILITIES Ongoing, proactive care management for high-risk, medically complex patients with primary focus on diabetes Use the nursing process of assessment, diagnosis, care planning, implementation, and evaluation to provide safe, therapeutic, and coordinated care for primary care patients during in-person and virtual interactions Support patient care transitions from recent hospital or emergency department discharges, specialty to primary care, and reduces barriers to care Partner with patients and families to develop individualized care plans, support disease self-management, and improve knowledge of their health conditions.
