Requisition #: req11227 Shift: Days FTE status: 1 On-call: No Weekends: No General Summary Under the direction of an Ambulatory Clinical Pharmacist, the Clinical Support Associate is a member of the health care team who performs clerical and clinical duties for the delivery of quality health care services in an ambulatory setting Requirements • High school degree (or GED) required. • Current Certification as Medical Assistant required • Basic Life Support required • Previous experience as Medical Assistant preferred • Ability to identify and solve problems, set priorities, and perform work within specified times with minimal direct supervision • Perform work accurately with an attention to detail and exercise sound judgment • Excellent interpersonal skills and ability to effectively work with the medical team • Ability to effectively communicate with patients and staff while demonstrating a high degree of diplomacy and tact • Knowledge of infection prevention & control standards and practices • Flexible, organized and able to function under stressful situations • Ability to understand, interpret and apply instructions, guidelines and procedures • Culturally sensitive and able to work with ethnically diverse patient populations • Ability to prioritize and work on multiple tasks • Intermediate experience with computers and information systems Essential Functions & Responsibilities • Evaluate and analyze panel reports and patient records to identify patients who need a referral to the ambulatory pharmacist for diabetes, hypertension, dyslipidemia, anticoagulation or a comprehensive medication review. • Perform virtual or telephonic outreach to schedule patients in need of follow up with the ambulatory pharmacist. • Provides patient with information and instructions as authorized by physician, advanced practice provider or ambulatory pharmacist. Processes messages from patients and front office staff to ambulatory pharmacist or other care team member. • Processes messages and call backs to patients, medical offices, patient assistance programs and pharmacies. • Provides clerical support which may include completing forms/requisitions, answering phones, pending refills, sending/receiving medical records, obtaining lab/x-ray reports, hospital notes, and referral information. • Documents all clinical activities accurately and completely in the EHR. • Participates in development, implementation, and maintenance of ambulatory pharmacy services. • Cooperates and participates in departmental and health system quality initiatives. • Participates in process improvement activities to enhance patient care and service. • Assists other department personnel in completion of their duties. • Attends and participates in the Ambulatory Pharmacy Workgroup and Clinical Pharmacist meetings as well as other programs, committees, meetings, and functions required by the health system or care team. • Performs other duties as assigned. These may include but are not limited to: Maintaining a current knowledge base of department processes, protocols and procedures, pursuing self-directed learning and continuing education opportunities, and participating on committees, task forces, and work groups as determined by management.
