Position Summary The DSCC Interim Relief Coordinator provides care coordination services to families eligible for the DSCC Interim Relief Statewide Program. Duties & Responsibilities • Provides care coordination services: • Assists families in collecting necessary medical and financial information necessary to determine eligibility for the Interim Relief Program. • Develops and implements a person-centered plan that is participant / family-centered, community-based and coordinated. • Monitors the person-centered plan activities. • Revises the person-centered plan to address the changing and ongoing concerns and priorities of the family through the comprehensive assessment. • Designs individual based intervention for participants and families. • Participates as a member of the multi-disciplinary care team: • Complies with University, Division, and HFS policy and procedures. • Provides discipline-based expertise to the multi disciplinary team. • Complies with state and federal regulations related to participants enrolled in the Interim Relief Program. • Promotes interagency collaboration and an organized network of integrated services: • Provides education to families, participants and providers regarding behavioral health services and strategies. • Participates in developing and/or implementing a networking plan for services needed for Interim Relief program participants. • Reviews the Interim Relief Services Application and Consent Form for eligibility review of all potential participants. • Participates in special projects impacting DSCC: • Participates in agency committees/projects on a statewide basis. • Performs other duties as assigned. • Attends all staffings on assigned participantsPerform other related duties and participate in special projects as assigned. Minimum Qualifications 1. Master's degree from an accredited school or university in social or behavioral science or a related health specialty area. Based on institutional requirements for each position: 2. A. Current State of IL Licensure as a Licensed Social Worker or a Licensed Clinical Social Worker. OR B. Three (3) years (36 months) of progressively more responsible full-time experience in social work in a medical/clinical or social service agency setting. 3. Qualified Mental Health Professional 4.
