Triple Ace Home Health Agency – Houston, TX Full-Time / Part-Time / PRN Triple Ace Home Health Agency, a growing and compassionate home health provider in Houston, Texas, is seeking a dedicated Licensed Vocational Nurse (LVN) to join our clinical team. We are committed to delivering exceptional patient-centered care in the comfort of our clients’ homes. Position Summary The LVN provides skilled nursing care to patients in their homes under the supervision of a Registered Nurse (RN) and in accordance with the patient’s plan of care. This position requires strong clinical judgment, excellent communication skills, and a commitment to high-quality home health services. Responsibilities • Provide skilled nursing care according to the individualized plan of care. • Perform patient assessments and report changes in condition to the supervising RN. • Administer medications and treatments as ordered. • Complete wound care, catheter care, injections, and other skilled procedures. • Document all care provided accurately and timely in the EMR. • Educate patients and families on disease management, medications, and safety. • Communicate effectively with the RN Case Manager, physicians, and interdisciplinary team. • Maintain compliance with state regulations, agency policies, and home health standards. • Provide compassionate, patient-centered care in the home environment. Requirements • Current Texas Licensed Vocational Nurse (LVN) license in good standing. • Minimum 1 year of clinical experience (home health required). • Current CPR certification. • Valid Texas driver’s license and reliable transportation. • Ability to travel within the Houston area. • Strong assessment and critical-thinking skills. • Excellent communication and documentation abilities. • Ability to work independently and manage time effectively. Preferred Qualifications • Prior Home Health experience (OASIS exposure is a plus). • Bilingual (English/Spanish) preferred but not required.
