Position Overview Under the direct supervision of the Network Director, the QIDP Coordinator provides mentorship and guidance to Qualified Developmental Disability Professionals (QIDPs), while ensuring timely and compliant completion of assessments and planning documentation for individuals supported. This role includes managing and overseeing all tasks related to QIDPs within the department, participating as a member of the Network’s leadership team, and representing the Agency in leadership and administrative functions. Salary: $64,000 year Hours: M-F 8:30am-5pm (3 days/week) 11am-7:30pm (2 days/week) Location: New Lenox, IL Reports To Network Director Network Or Department Network 1 Essential Functions • Oversee and direct the daily tasks and responsibilities of QIDPs, with the number of QIDPs varying based on departmental needs. • Review QIDP applications and interview potential candidates, under the direction of the Network Director. • Make recommendations to the Network Director on hiring, termination, promotion, and evaluation of QIDPs. • Review clinical documentation submitted by QIDPs to ensure compliance with state and federal standards as well as Trinity’s quality expectations; provide feedback as needed. • Ensure timely completion of all assessments, planning documentation, and related meetings, including Flourish Celebrations. • Monitor systems to ensure QIDPs are training staff on planning and tracking documentation. • Promote integrity and consistency throughout the Flourish Planning process to ensure individuals are supported in leading full, abundant lives. • Communicate professionally and promptly with parents, guardians, staff, visitors, community agencies, and internal/external entities. • Collaborate with Trinity staff to suggest events or system improvements and proactively identify and report potential issues to the Network Director. • Meet regularly and as needed with the Network Director to discuss team needs, barriers, and person-specific concerns related to quality care (e.g., medical or behavioral needs). • Complete Council on Quality and Leadership (CQL) training to become a Certified Personal Outcome Measure (POM) Interviewer, serving as a mentor on POM interviews, data application, and outcome achievement. Skills And Abilities • Strong verbal and interpersonal communication skills. • Consistent professionalism in interactions with staff, people supported, families, visitors, and external entities. • Effective advocacy for people supported; able to lead both internal and external team meetings. • Ability to develop, implement, and evaluate program goals, objectives, policies, and procedures. • Clear communication of policies, procedures, and reports to relevant stakeholders. Physical Demands • Lift over 40 lbs from floor to waist unassisted. • Push/pull up to 75 lbs and maneuver wheelchairs weighing up to 200 lbs. • Operate motor vehicles. • Ability to remain seated or standing for extended periods. • Frequent activities include driving, lifting, kneeling, crouching, reaching, and repetitive motion. Qualifications • Bachelor’s degree in Psychology or related human services field. • Minimum of two years of experience working with people with intellectual/developmental disabilities.
