Candidates must have a high school diploma or equivalent and relevant experience in child behavioral/mental health, developmental disabilities, or substance use. A valid driver's license and residency in North Carolina are also required.

What We’re Looking For Trillium Health Resources has a career opening for a Rapid Access Care Coordinator to join our team! The Rapid Access Care Coordinator (RACC) is responsible for supporting youth from admission into the program through the initial seven (7) days of safety and stability. Rapid Access Care Coordinators are qualified professionals who hold knowledge in the entire service array for Behavioral Health and IDD (excluding Intermediate Care Facility- Intellectual Disability/ICF-ID services and Innovation Waiver services). RACC will work closely with Trillium’s DSS Liaison, DSS staff, DJJ Liaison, DJJ staff, legal guardian, medical professionals, and others involved in the well-being of DSS/DJJ-involved youth. On a typical day, you might: * Conduct a comprehensive review of members’ records via Trillium Software Platforms. The Care Coordinator will review available benefits and identify resources. * Engage in intensive care coordination upon admission through the first initial days of safety/stability. * Collaborate with integrated care team members, both internal and external stakeholders, to provide integrated care. * Review member information within the platform, update information as needed, and/or inform the appropriate staff when inaccuracies are found. * Identify potential barriers to stable/safe settings and assist with the reduction of those barriers by linking to available services, resources, providers, etc. Employee Benefits: Trillium knows that work/life balance is essential. Preferred: * Experience providing crisis intervention or crisis response to individuals within the behavioral/mental health, developmental disabilities, or substance abuse population. * Work experience with youth who are transitioning to adulthood.
