A Master's degree in a human services field and three years of experience with the BH/MH/SU/IDD population are required. Candidates must be fully licensed in North Carolina and have a valid driver's license.

What We’re Looking For Trillium Health Resources has a career opening for a Population Health Manager - Inpatient Discharge Planning Team to join our team! The Population Health Manager - Inpatient Discharge Planning Team is responsible for providing administrative oversight and clinical supervision to the Inpatient Discharge Planning Team, a cross functional team of staff who perform tasks as liaison between Trillium and the local community hospitals and providers. The Population Health Manager - Inpatient Discharge Planning Team will be available to perform clinical triage, lead on emergency or crisis calls/contacts or provide guidance and support when needed or requested. On a typical day, you might: * Monitor and oversee the activities of the Inpatient Discharge Planning Team to ensure member and stakeholder needs are addressed appropriately, expeditiously and in a courteous manner. * Monitor and evaluate staff performance, conduct new employee orientation, and trainings. * Review and analyze hospital data to identify barriers to discharge and communicate barriers appropriately. * Review program data to ensure performance measures and benchmarks are consistently met. * Communicate and disseminate pertinent information to internal and external stakeholders. * Review, revise or develop procedures, processes, protocols, workflows and training material. Preferred: * Supervision of two (2) or more employees that provide MH/SU and or IDD services. * Experience working with acute care discharge planning and/or utilization management including mental health and physical health care needs. * Experience coordinating physical healthcare needs.
