What We’re Looking For Trillium Health Resources has a career opening for a CRISP Child Care Manager to join our Care Management team! The CRISP Child Care Manager is responsible for providing care coordination targeting those with chronic, unresolved or complex physical, behavioral health, and determinant social needs. This includes providing care planning with foundations in national evidence-based and informed standards to do whole person care. The CRISP Child Care Manager is expected to assess and respond to members’ health and safety needs when the Trillium Catchment is under a state of emergency or weather advisory, or when health and safety needs are identified for members. On a typical day, you might: * Engage members in care coordination/care management process. * Provide complex care management, in compliance with NCQA and any other identified regulatory/required accreditation standards, to assigned members who may have identified needs with behavioral health, physical health, co-occurring, co-morbid, or multi-morbid conditions. * Provide education about and linkage to as needed, all available services, natural and community supports, treatment options, diagnostic services, etc. * Linkage to needed psychological, behavioral, educational, physical, and dental evaluations. Identifies, links, or, if appropriate, facilitates referrals for health-related services, community-based resources, school/academic needs, and social support services. * Ensure the coordination of care with each member’s primary care physician and/or other identified specialist/dental provider.
