As a Nurse Case Management Lead Analyst, you will play a pivotal role in guiding patients through their care journey, ensuring they receive the right support at the right time. Responsibilities Lead strategic initiatives to improve patient care coordination and optimize resource utilization. Analyze complex clinical data to identify trends and recommend actionable solutions that enhance quality of care. Collaborate with cross-functional teams to design and implement innovative case management programs. Drive continuous improvement by monitoring outcomes and adjusting strategies to meet evolving patient needs. Serve as a trusted advisor, providing guidance and mentorship to team members to foster professional growth. Required Qualifications Registered Nurse (RN) license in good standing. Possess license in state of residence. Minimum 5 years of experience in case management or related clinical role. Proven ability to analyze data and develop strategic care plans. Strong communication and collaboration skills. Preferred Qualifications Bachelor’s degree in Nursing. Experience with healthcare analytics and population health management. Certification in Case Management (CCM or equivalent). If you will be working at home occasionally or permanently, the internet connection must be obtained through a cable broadband or fiber optic internet service provider with speeds of at least 10Mbps download/5Mbps upload. For this position, we anticipate offering an annual salary of 76,600 - 127,600 USD / yearly, depending on relevant factors, including experience and geographic location.
