Calling All Care Coordinators! Team Wellness Center is one of the largest integrated healthcare providers in Wayne County, Michigan, with outpatient clinics in Detroit and Southgate. Accredited as a Behavioral Health Home and Certified Community Behavioral Health Clinic, we are leaders in integrating physical health, mental health, substance abuse treatment, and social services. Serving more than 10,000 members each year, we pride ourselves on providing compassionate wraparound care through an interdisciplinary team of doctors, nurses, psychiatrists, therapists, peer specialists, and care coordinators. Position: Care Coordinator (BSW or related field) We are seeking passionate and dedicated Care Coordinators to join our team. This role provides direct support to members by coordinating services, linking them to resources, and ensuring their overall treatment plan is carried out effectively. You will work with clients experiencing a wide range of needs including mental health, substance use, housing, employment, and community integration. Responsibilities include: • Collaborate with clinical teams to support member treatment plans • Provide care coordination for individuals with depression, severe mental illness, and substance use disorders • Link members to insurance, housing, food, employment, and other community resources • Assist with family reunification and social integration • Drive members to appointments in your personal vehicle (required) • Maintain accurate documentation in electronic health records • Ensure members are safe, socially thriving, and supported in recovery Qualifications: • Bachelor’s degree in Social Work (BSW) or related field • Eligible for licensure in the State of Michigan; must obtain professional license within 90 days • Valid driver’s license, reliable personal vehicle, and auto insurance (required for member transportation) • Strong communication, organization, and teamwork skills • Compassion for working with vulnerable populations We Offer: • Medical, dental, and vision insurance • Paid vacation • 8 paid holidays and floating paid days off • Bonus opportunities • Tuition reimbursement • Retirement planning and additional benefits Job Type: Full-time Work Location: In person Job Type: Full-time Pay: $35,568.00 per year Benefits: • Dental insurance • Health insurance • Paid time off • Tuition reimbursement • Vision insurance Work Location: In person
