Position: Care Coordinator (BSW or related field) We are seeking passionate and dedicated Care Coordinators to join our team. This role provides direct support to members by coordinating services, linking them to resources, and ensuring their overall treatment plan is carried out effectively. You will work with clients experiencing a wide range of needs including mental health, substance use, housing, employment, and community integration.
