Organization: PAMF-Palo Alto Medical Foundation ALD Position Overview: The Assistant Case Manager works in collaboration with the Physician, Psychotherapist, consulting Psychiatrist, and other care team members to assure the appropriate level of care and coordination for outpatient services. The Assistant Case Manager reports to the Clinical Manager of Pediatric Integrated Behavioral Health. The Assistant Case Manager has frequent contact with patients, families, physicians, the interdisciplinary team, schools, ancillary services, and community resources. This position is responsible for gathering and reporting patient functioning data and metrics, entering data into the patient care registry, communicating between collaborative care team members, direct patient contact to assigned patients and family members, and emotional support for patients and families, to include crisis intervention and de-escalation where required. Job Description: This position will be onsite in our Dublin facility 2 days/week, onsite 1 day in our Livermore facility, and 2 days remote. EDUCATION: Bachelor's: BA or BS in psychology, social work, or a related field. TYPICAL EXPERIENCE: 2 years' recent relevant experience. SKILLS AND KNOWLEDGE: • Oral and written communication skills. • Interpersonal and time management skills • Ability to work effectively and prioritize tasks in a fast-paced environment with rapidly shifting priorities and competing demands. • Ability to work independently with a minimum of direction. • Intermediate PC skills and word processing skills required. Pay Range is $35.35 to $45.95 / hour The salary range for this role may vary above or below the posted range as determined by location. This range has not been adjusted for any specific geographic differential applicable by area where the position may be filled. Compensation takes into account several factors including but not limited to a candidate’s experience, education, skills, licensure and certifications, department equity, training and organizational needs.
