Full job description Job Overview We are seeking a dedicated and compassionate Home Health Case Manager to coordinate and oversee patient care in a home health setting. This role involves managing complex cases, ensuring seamless communication between healthcare providers, patients, and families, and facilitating effective discharge planning. The ideal candidate will have extensive clinical experience in acute and post-acute care environments, with a strong understanding of medical terminology, patient assessment, and utilization management. The Home Health Case Manager will play a vital role in delivering high-quality, patient-centered care that promotes recovery and well-being. Duties Coordinate with multidisciplinary teams across inpatient, outpatient, and post-acute care settings to ensure continuity of care. Manage utilization review and case management processes to optimize resource use while maintaining quality standards. Oversee diagnostic evaluations, medical records documentation, and compliance with HIPAA regulations. Facilitate communication between hospitals, clinics, managed care organizations, and home health agencies. Monitor patient progress through regular follow-ups and adjust care plans accordingly. Educate patients and families about treatment options, medication management, and health maintenance strategies. Ensure adherence to infection control protocols and safety standards during patient interactions. Document all patient interactions accurately in medical records and ensure compliance with healthcare policies. Experience Experience in EMR systems Knowledge of medical terminology, physiology, HIPAA compliance, and medical records management is essential. Proven ability to perform patient assessments including vital signs and comprehensive clinical evaluations. Experience with managed care programs and post-acute care coordination is beneficial.
