What You'll Do: - You will engage with patients to understand their needs and guide them through the care access process. - You will coordinate with clinical and administrative teams to schedule appointments and follow-ups. - You will provide clear and empathetic communication to patients regarding their care options. - You will document patient interactions accurately in the health network’s systems. - You will assist in resolving patient concerns and escalating issues when necessary. - You will support weekend operations by maintaining efficient workflow and collaboration. - You will contribute to continuous improvement initiatives aimed at enhancing patient engagement. What You Bring: - You bring at least 2 years of experience in patient engagement, healthcare coordination, or a related field. - You have strong communication and interpersonal skills to effectively interact with diverse patient populations. - You possess proficiency with electronic health records (EHR) and scheduling software. - You demonstrate problem-solving abilities and attention to detail in a fast-paced environment. - You have the ability to work independently during weekend shifts with minimal supervision. Bonus Points If You Have: - Experience working in a hospital or healthcare network setting. - Knowledge of healthcare regulations and patient privacy standards (HIPAA). - Bilingual skills to support non-English speaking patients. - Certification in patient navigation or healthcare coordination.
