Role Overview: As a Weekend Patient Engagement Partner in the Connect to Care Access Center, you will play a vital role in ensuring patients have seamless access to healthcare services during weekend hours. Your purpose is to engage with patients, coordinate care, and support the network’s mission of excellence in patient satisfaction and health outcomes. What You'll Do: - You will engage with patients to understand their healthcare needs and provide timely assistance. - You will coordinate appointments and follow-up care to ensure continuity of services. - You will collaborate with clinical and administrative teams to facilitate patient access. - You will document patient interactions accurately in the health network’s systems. - You will address patient concerns and escalate issues as necessary to appropriate departments. - You will support weekend operations by maintaining efficient workflow and communication. - You will contribute to quality improvement initiatives focused on patient engagement. What You Bring: - You bring at least 2 years of experience in patient engagement, healthcare coordination, or a related field. - You have strong communication and interpersonal skills to effectively interact with diverse patient populations. - You possess proficiency with electronic health records (EHR) and scheduling software. - You demonstrate problem-solving abilities and attention to detail in a fast-paced environment. - You have a compassionate approach and commitment to patient-centered care. Bonus Points If You Have: - Experience working in a hospital or health network access center. - Knowledge of healthcare regulations and patient privacy standards (HIPAA). - Bilingual skills to support diverse communities. - Certification in patient navigation or healthcare coordination.
