Role Overview: As a Weekend Patient Engagement Partner in the Connect to Care Access Center, you will play a crucial role in ensuring patients receive timely and compassionate support during weekend hours. Your efforts will help facilitate access to care, improve patient satisfaction, and support the overall mission of St. Luke's. What You'll Do: - You will engage with patients to understand their needs and provide appropriate guidance and support. - You will coordinate with clinical and administrative teams to facilitate patient access to care services. - You will manage patient inquiries and resolve issues efficiently during weekend shifts. - You will document patient interactions accurately in the system to maintain up-to-date records. - You will assist in scheduling appointments and follow-up care as needed. - You will contribute to continuous improvement initiatives by providing feedback on patient engagement processes. - You will maintain confidentiality and adhere to all healthcare regulations and organizational policies. What You Bring: - Minimum of 2 years experience in patient engagement, customer service, or healthcare support roles. - Strong communication and interpersonal skills to effectively interact with diverse patient populations. - Ability to work independently and as part of a team during weekend shifts. - Proficiency with electronic health records (EHR) systems and basic computer skills. - Compassionate and patient-centered approach to care. Bonus Points If You Have: - Experience working in a healthcare access center or call center environment. - Knowledge of healthcare regulations such as HIPAA. - Bilingual abilities to support diverse patient communities. - Certification in patient navigation or related healthcare fields.
