Role Description We are seeking a compassionate and skilled Wellness Nurse to join our Memory Care team. This is a full-time, on-site position located in Johns Creek. The Wellness Nurse will oversee and support health care needs, supervise the medication program, and monitor residents' health and well-being. Responsibilities include conducting assessments, creating individualized care plans, collaborating with caregivers and families, and ensuring compliance with all regulations relevant to memory care. The role also involves offering emotional support and ensuring residents' dignity and quality of life. Qualifications • Proficiency in providing compassionate, patient-centered care in a memory care or assisted living setting. • Strong clinical skills, including medication management and conducting health assessments. • Exceptional communication and interpersonal skills for interacting with residents, families, and interdisciplinary teams. • Knowledge of state and federal regulations relevant to assisted living and memory care communities. • Ability to document and maintain accurate health records and care plans. • Licensed Practical Nurse (LPN) or Registered Nurse (RN) certification is required, with a valid license to practice in Georgia. • Experience in dementia care or senior living is preferred but not required.
