Role Overview: As a Housing Management Specialist, you will play a critical role in supporting the Medical Management/Health Services team by ensuring that housing-related needs are effectively managed to improve health outcomes. You will work closely with clinical professionals and community partners to coordinate housing resources and services for our members. What You'll Do: - You will assess and address housing needs that impact member health and well-being. - You will collaborate with healthcare providers and community organizations to coordinate housing support services. - You will monitor and manage housing-related cases to ensure timely and effective interventions. - You will maintain accurate records and documentation related to housing management activities. - You will provide guidance and support to members navigating housing challenges. - You will contribute to the development of housing policies and procedures within the healthplan. - You will analyze data to identify trends and opportunities for improving housing support services. - You will participate in team meetings and training to stay current on best practices. What You Bring: - Minimum of 3 years of experience in housing management, social work, or a related field. - Strong knowledge of housing resources, community services, and healthcare integration. - Excellent communication and interpersonal skills to work effectively with diverse stakeholders. - Ability to manage multiple cases and prioritize tasks in a fast-paced environment. - Proficiency with case management software and Microsoft Office Suite. - Strong problem-solving skills and attention to detail. Bonus Points If You Have: - Experience working within a healthcare or managed care environment. - Familiarity with Medicaid and Medicare housing support programs. - Certification in social work or housing management. - Bilingual abilities, particularly in Spanish.
